Your selection for gender doesn’t seem to have any effect on your account; it’s most likely used for demographic tracking purposes by YouTube, allowing them to analyze the viewing habits of men and women.
If you want your friends to be able to find your YouTube account by your email address, check the appropriate box. This will allow your friends to see that you’ve joined YouTube and add your profile as a friend.
There’s one more box to check: the one that determines whether or not you’ll receive updates from YouTube. As the site already sends you a fair amount of email, I’d recommend keeping this box unchecked. Finally, click on the “I Accept” button at the bottom of the page, which signifies that you’ve read YouTube’s Terms of Service.
